PROGRAM GUIDELINES

Our Dental Membership Program is not insurance. The program is offered to individuals and families who do not have insurance, or who choose not to utilize their insurance. Dental Membership Program benefits are not transferable to another dental practice or dental specialty practice.

Our affordable plan includes the following services at no extra charge:

Concierge Patient Care

New Patient & Six Month Recall Exam

Annual Digital X-Rays (4 Bitewings And 2 Periapical 1 Set Per Year)

Two Preventative Teeth Cleanings (in the absence of periodontal disease)
